The minutes record (AQL-F05) is laid out so the minutes capture outputs as they are made: attendance; the status of previous actions with a decision on each; each input with what the data showed and the chair's conclusion; the four conclusions on the quality system; every output with an owner, a date and its type; the resource decisions with their cost or headcount; the changes to the quality system; the actions on risks; and the next review and the chair's signature.
- Fill in the header and copy the open actions from the register before the meeting.
- Write in the room, on a screen everyone can see, so decisions are corrected while everyone is there.
- Record the conclusion for every input, even when no action is needed.
- Within two working days, enter every output in the action register under the same reference.
- Issue the minutes, signed by the chair, within five working days, filed with the pack and its record index.
