The Quality Performance Kit HandbookChapter 1 of 20

Why quality reporting fails

Every shop we have worked with reports something about quality every month. The trouble is rarely a lack of data. It is what happens between the data and a decision. The same patterns show up everywhere:

  • Measures without definitions. On-time delivery means the promise date to one person and the customer's request date to another; NCRs include supplier rejects one month and not the next. The trend line moves because the definition did.
  • Percentages averaged instead of added up. Three months at 90, 95 and 98 percent is not a 94 percent quarter if the volumes were different. Someone in the review will notice, and the whole report loses credibility.
  • Every bad month treated as a signal. Counts move by chance. A shop that opens a corrective action every time the NCR count rises chases noise all year and misses the slow change that matters.
  • Totals that hide the problem. The total NCR count falls while one shift, one supplier or one operation gets steadily worse. Nobody splits the total, so nobody sees it.
  • Counts with no dollars. Management hears NCRs and escapes and decides in money. Without a cost figure, quality competes for resources with one hand tied.
  • Objectives set in January and read in December. A target with no plan, no owner and no quarterly check is a wish, and everyone in the building knows it.
  • Reports that end without a decision. Forty charts are shown, everyone nods, and the meeting closes with nothing assigned. Next month the same charts come back, slightly worse.

None of these is fixed by a better chart. They are fixed by a short list of defined measures collected the same way every month, a few plain rules for reading them, a cost figure management recognizes, and a monthly meeting that ends with owners and dates. That is what this kit gives you.
