The Customer Complaint Response Kit HandbookChapter 9 of 21

Writing the response

The response is what the customer's supplier quality engineer reads, judges and files. Write it from the investigation record and nothing else, in the structure of letter 3 (AQL-B15): what happened, containment and correction, root cause, escape cause, corrective actions with owners and dates, other places reviewed, and how effectiveness will be proven. Where the customer requires its own 8D form or portal, use it, and put the same content in its boxes.

If the cause is not confirmed by the customer's date, send an interim response on the date: what is known, what is not, what has been done, and when the next response comes. An on-time interim response is better than a late final one.

Why customers send responses backWhat fixes it
No escape cause, or 'added 100 percent inspection' as the escape actionA section on the escape point with its own action on the plan, the gauge or the method; inspection kept as containment with an end date.
A root cause that stops at a person ('operator error', 'retrained')Ask what about the job made the error possible, and act on that. Retraining supports an action; it is rarely the action.
A cause with no evidenceName the trial, the data or the reproduction that confirms it.
Actions with no dates, owners or evidenceA table: action, aimed at, owner, date, evidence.
Nothing on other parts or processesA section on where else the cause could act, and what was checked.
No effectiveness planThe measure, the window and the result that will show it worked.
Numbers that differ from earlier lettersWrite every letter from the same record; check before sending.
