The CAPA Command Kit HandbookChapter 6 of 15

Investigation

Pick the method for the problem, not for the form (B04). A single chain of events gets a careful 5 Whys (B06). Several possible causes get a fishbone (B07) and a test of each against the data (B05). Customer escapes, repeats, safety issues and problems that cross departments get a full 8D with a team (B03).

Every investigation ends with two causes: the root cause, why the problem was made, and the escape cause, why it wasn't caught. Neither is accepted until it is confirmed with data and names a condition of the process, not a person.
