8D Problem-Solving Workbook
One tab per discipline, D0 to D8, rolled up to a status page: an is / is-not table, containment that is verified, root and escape causes confirmed with data, an effectiveness criterion set before the actions go in, and prevention carried to similar processes.
Free to use, change and copy inside your own organization. Editable Excel, plus a fillable PDF you can complete on screen.
The problem it solves
An 8D is supposed to be a team working a problem from the emergency response through to proven prevention. Too often it is a form filled in at the end, with containment that was never checked, a root cause nobody tested, an action judged after the fact, and no thought for the other parts and suppliers with the same weakness. This workbook gives each discipline its own tab with the checks that matter, rolls owners and dates up to one status page, and checks the record across tabs, so an 8D cannot read complete while the work behind it is missing.
Who uses it, and when
Quality and supplier quality engineers who lead 8Ds, the champions who sponsor them, and the customers and suppliers who take part. For customer escapes, repeats, safety-related problems and anything that crosses departments or reaches a supplier. Open it the day the problem arrives and keep it open until D8.
