Resource Library/Quality Leadership, Metrics and Management Review/Risks and Opportunities Register
Risks and Opportunities Register
One register for the quality system's risks and opportunities: each written as 'if ... then ...', rated on scales you set, treated with an owned action, re-rated afterward and checked, with a heat map and a summary for management review.

The problem it solves
In many small aerospace shops the risk register was written for the certification audit and not opened since. Its lines are single words, the ratings never change, actions have no dates, and nobody checks whether an action reduced the risk. Meanwhile the real risks (the single-source anodizer, the setup technician who left, the drawing revision released mid-job) show up first as escapes. This register keeps each risk and opportunity as a sentence with a cause and an effect, rates it on scales you set, records the treatment and an owned, dated action, re-rates it after the action, and says on every line what it needs next.
Who uses it
Quality directors and managers who own the register, the process owners who own each line, and top management, who review it.
When
Review it in full at the annual review; take the high risks and the overdue actions to every management review; add or re-rate a line the week something changes.
