Operator Qualification and Authorization Record

The evidence behind every stamp, release and signature: a qualification record for each person and task, a register that shows which authorizations are current, due, lapsed or expired, and a stamp log that keeps every stamp traceable to one person.

The problem it solves

Stamps and release authority are where competence and product acceptance meet. In many shops the stamp was issued years ago on a supervisor's word, the vision exam lapsed last spring, the person has not done the task in eight months, and a stamp belonging to someone who left is still in a drawer. None of it shows until an auditor or a customer asks who released a part and on what basis. This workbook records the evidence each time someone is qualified, keeps every authorization on one register with its requalification, vision and lapse dates, and logs every stamp from issue to retirement.

Who uses it

Quality managers and supervisors who grant authorizations, and the people who hold them.

When

Fill a qualification record each time someone is qualified or requalified; keep the registers current as authorizations are granted, used, suspended and returned; review the Summary monthly and before audits.
