Retraining After a Quality Failure Record
Decide whether retraining is the right response to a quality failure before anyone is retrained, tie it to the corrective action that changes the job, and prove the training worked on the job. Retraining here supports a corrective action; it never stands in for one.

The problem it solves
'Operator retrained' is the most common corrective action in aerospace manufacturing and one of the least effective. It appears when the investigation stops at the person who was there. The condition that made the error possible, an instruction with a gap, a file structure that offers the wrong program, a check that cannot catch the mistake, stays exactly as it was, and the same error comes back on another shift with another person. Customers and auditors know the pattern and send such responses back. This record puts a gate in front of retraining: it asks whether the failure really was a gap in knowledge or skill, sends anything else back to the corrective action, and when training is right, links it to the system change, trains everyone who does the task, and checks on the job that it worked.
Who uses it
Supervisors and quality engineers who respond to a nonconformance involving people, and the corrective action owner.
When
Whenever retraining is proposed after a nonconformance, an escape, an audit finding or a near miss, and before the training happens.
