Supplier Performance Improvement Plan
The written agreement with a supplier that is falling short: the gap in numbers, the causes, the actions with owners and dates, the milestones, what you will provide, the exit criteria and the consequences, signed by both sides and reviewed to closure.

The problem it solves
When a supplier's delivery or quality slips, most shops send a stern email, ask for 'a plan', and get back a page of good intentions with no dates. Nobody agreed what 'better' means, so the plan cannot fail and cannot succeed; months later the supplier is still late and the conversation starts again. This template turns the conversation into an agreement: the gap stated in numbers both sides accept, the causes found by the supplier, actions with owners and dates, monthly milestones that show whether the plan is working, what you will do to help, the exit criteria, and what happens if the plan is missed. Both sides sign it, and every review is recorded on it until it closes.
Who uses it
Supplier quality engineers who write and run the plan, with the supplier's senior manager who owns it, and your purchasing lead.
When
When a supplier reaches Level 2 on your escalation matrix (AQL-C10), or whenever performance has been below target long enough that a corrective action request alone will not fix it. For a supplier in deep trouble, use the 30/60/90-day recovery planner (AQL-C12) alongside it.

A management aid, not certification or a registrar audit. Using it does not by itself show conformity to AS9100D or any other standard.
