Corrective Action Request (CAR) Template
The request a quality team sends to a supplier or an internal department: the nonconformance stated so it can't be argued with, a due date for every stage, and a decision on each answer with the reason.
Free to use, change and copy inside your own organization. Editable Word, plus a fillable PDF you can complete on screen.
The problem it solves
A CAR that goes out vague ('parts out of spec, please respond') comes back vague ('operator retrained, will monitor'). With one due date for the whole answer, the response arrives weeks later in a single block; the issuer accepts or rejects all of it, often with no reason written down; and the CAR is closed when the paperwork is in, not when the problem has stopped. This template states the nonconformance with its requirement and evidence, asks for containment by a date, splits the answer into stages that each have their own due date, and gives the issuer a place to accept or reject each stage with the reason, so a weak answer is caught early and the record shows why.
Who uses it, and when
Supplier quality engineers, quality engineers and quality managers who issue CARs, and the supplier or department lead who answers one. When a nonconformance needs the party responsible for it to find and remove the cause: a supplier lot rejected at receiving, a repeat internal nonconformance, an audit finding assigned to a department, or a customer escape traced to a supplier.
