CAPA Effectiveness Verification Worksheet
Prove a corrective action worked, not just that it was done: separate containment, correction and corrective action, set a measurable criterion before the action goes in, and hold the record open until the evidence is in.
Free to use, change and copy inside your own organization. Editable Excel, plus a fillable PDF you can complete on screen.
The problem it solves
Most corrective actions are closed when the action is done, not when it is shown to have worked. Months later the same nonconformance comes back under a new number, and nobody connects the two. This worksheet makes the difference between doing an action and proving it worked hard to miss: each record carries a measurable criterion set before the action starts, a verification window, the evidence observed, and a closure that reads 'Not yet' until that evidence is in.
Who uses it, and when
Quality engineers and managers who own corrective actions, and whoever authorizes closure. Open a record when a corrective action is raised, write the criterion before the action is implemented, come back when the window ends, and review the register at every CAPA meeting.
