The problem it solves

Transitions rarely fail on the standard's content. They fail on the things around it: the final text arrives later or reads differently than the draft, the one person who holds the plan is out, suppliers never answer the letters, the IT contractor's hours were never budgeted, and the registrar has no audit date left when you finally want one. Each of these is predictable, and most can be reduced months ahead if someone has written them down. This register lists the risks to the transition, rates them on a plain grid whose thresholds you set, and gives each one an owner, actions, an early warning sign and a review date, so management review sees them before they become findings.

Who uses it

Quality directors and the quality manager running the transition, with the plant manager who owns the resources.

When

Once now, alongside the planning matrix (AQL-H02); reviewed monthly while the plan runs; at each management review; and again when the final text is published.
