Product Safety Reporting and Escalation Matrix
Decide before the event: for each kind of product safety event, what to stop and hold, who inside is told and by when, who decides, when the customer hears and what goes outside. Then log every real event against those times.

The problem it solves
When a product safety event happens in a small shop, the first hour decides most of what follows: whether product keeps running, whether the right person hears about it, and whether the customer learns of it from you or from their own receiving inspection. Most plants have no written answer to 'who do I call, and how fast?' beyond the quality manager's mobile number. This workbook writes the answer down by kind of event, names the people and their backups, and logs each real event against the times you set, so you can show an auditor, a customer and yourselves that the escalation works.
Who uses it
Quality leaders who own product safety and the plant manager who signs off the matrix; supervisors, inspectors and engineers who use it on the floor.
When
Write the matrix once with the plant manager, check it against each contract, post the first-hour card, log every event the hour it is found, and review the Summary monthly and at management review.
