The problem it solves
Org charts say who reports to whom, not who signs off an MRB disposition, who decides that a nonconformance needs a corrective action, or who recalls a gauge past its date. When those answers live in habit, two people each assume the other has it, and an auditor's question about authority lands on whoever is in the room. This matrix writes each answer down, one line per role and activity, and checks that every activity has exactly one accountable owner.
