An internal audit program should be the plant's own early warning: the place where a weak process shows up months before a customer escape or a registrar's major. In practice the program often turns into a ritual that finds nothing. The causes are the same in almost every shop we have worked in:
- Planned by calendar, not by risk. Every department gets one audit a year in the same month. The process that caused last quarter's escape gets the same attention as the mail room.
- Audited from the desk. The auditor reads the procedure, asks whether it is followed and checks a few records the auditee chose. Nobody follows a real job, picks up a gauge or visits second shift.
- Sampled from the list. Gauges are checked from the recall list, people from the training matrix. Anything never on the list can't be found.
- Findings argued away. Vague evidence turns every finding into a negotiation, and classification follows the mood of the closing meeting.
- Responses that fix the instance. The gauge is calibrated, the document replaced, the person retrained, and the record closed. The way it happened is still there.
- No follow-up. Nobody looks again at the process to see whether the fix held, so repeats are written as new minors.
- One auditor, overloaded. The quality manager audits everything, including their own processes, and the schedule slips every time production gets busy.
None of these is solved by a better checklist alone. They are solved by a program that chooses where to look, auditors who look at the work, findings written on evidence, responses that remove causes, and a rhythm that keeps all of it moving. That is what this kit gives you.
