The problem it solves
On audit day most of the stress, and many of the findings, come from evidence that is slow to find, held by someone who is away, or not quite what the auditor asked for. Afterward nobody can say exactly what was handed over. This register fixes both ends. The evidence map lists, clause by clause, the evidence an auditor asks for, where it lives, who holds it and when it was last opened and checked. The request log records every request during the audit, the records provided, the minutes it took and how the auditor received it, so the slow spots and the near-misses are fixed before the next audit.

When
Build the map once and check it every quarter, and in the six weeks before any external audit. Use the request log during every external audit and every internal audit of more than half a day.
