The problem it solves
Internal audit reports are often either a checklist with ticks or a paragraph that says 'process generally effective, two findings raised'. Neither tells the process owner what to fix and why, and neither shows a registrar what the audit actually examined. Findings without evidence get disputed, earlier findings are never followed up, and the program can't show it is finding real problems. This template sets out a report in the order a reader needs it: what was audited, the conclusion, the findings with their evidence and type, what works well, the follow-up of earlier findings, and the samples behind it all.
Who uses it
Lead auditors writing the report; process owners and top management reading it; quality managers who keep the program's records.
When
Write it from the checklist (AQL-A05 or AQL-A07) after the closing meeting, and issue it within ten working days. File it with the audit plan and checklist.
